Important service update
We are currently receiving a high number of enquiries regarding the meningitis B outbreak and other immunisations.
For children under 16 years old: Please allow time for us to respond to your query. Please note that we are not a clinical service and cannot give advice. If you need advice, please contact your GP. GPs can contact us directly for information if they do not have it.
For young people aged 16 to 19: We cannot share this information with parents and carers. The young person must either contact us directly for their vaccination records, or contact their GP. If they need advice, they should contact their GP. GPs can contact us directly for information if they do not have it.
For those aged 20 and over: We do not hold information for young people over 19, so all queries should be made to the GP.

CHIS Healthcare Professional Portal

The SCW Child Health Information Service Healthcare Professional Portal (CHIS HPP) is a web based application that as been designed for use by Health Visiting, School Age Immunisation and GP Practice Teams. The Portal has been developed to provide bespoke views for each healthcare discipline.

The CHIS HPP enables healthcare professionals to view individual child’s demographic, screening and vaccination information held in the CHIS IT system to support the delivery of UKHSA Screening and Immunisation Programmes. The Portal also provides dashboards that enable healthcare professionals to identify easily the children that require vaccination.

Access to the Portal is via a SMART Card so there is no need to remember additional passwords. The Portal is currently live in the Surrey and Thames Valley Regions. If you require access, please email This email address is being protected from spambots. You need JavaScript enabled to view it.
